Our 18 month old does not like to keep hats on his head, I think he finds them too tight and cumbersome . We went out the other day though and this stayed on through out! Its a really soft fabric and also looks really cute and grown up The size I got was a 0-3 I would say a 3yr old perhaps go up a size, but our son does have quite a big head :D
